South Western Sydney Primary Health Network (SWSPHN) in partnership with Outcome Health, has worked over the last two years to build an extensive suite of Quality Improvement in Primary Care (QIPC) reports. The aims of QIPC reports include:
- Embed QI systems for improved data quality and analysis of GP practice data whilst identifying further improvement opportunities;
- Assist with goal-setting and provide progress reports and feedback;
- Champion QI activities to drive improved clinical care and better patient outcomes in general practice.
Through an extended partnership with all POLAR PHNs, Outcome Health will shortly release the latest installment in the QIPC journey – Cardiovascular Disease (CVD), and Chronic Kidney Diseases (CKD). These new reports will help practices better screen and manage their patient cohorts for each of these chronic conditions.
POLAR reporting is simple and easy to use, enabling practices to track progress on their QI journey. The new modules will be available to all POLAR practices later this month.
