Through this partnership, Capital Health Network will support general practices across the ACT region with enhanced population health analytics, quality improvement capabilities, and data-informed service planning through POLAR.

POLAR provides practices and PHNs with meaningful insights into healthcare trends and patient outcomes, helping clinicians and practice teams make informed decisions that improve care delivery and operational planning. The platform also supports practices participating in Practice Incentive Payment – Quality Improvement (PIP QI) activities.

By joining the POLAR network, Capital Health Network continues its commitment to supporting digitally enabled, evidence-based primary healthcare across the ACT.
