Outcome Health is very pleased to announce the newest member of the ever-growing POLAR family – North Western Melbourne Primary Health Network.

With support from Outcome Health, North Western Melbourne PHN has partnered with POLAR to support general practices across the region in strengthening their use of data-driven quality improvement and population health insights.

North Western Melbourne PHN recognises the value of POLAR’s innovative platform in helping clinicians provide enhanced support to patients, while also enabling improved business planning and service delivery within practices. POLAR will also assist practices to meet data submission requirements for Practice Incentive Payment – Quality Improvement (PIP QI) program.

How North Western Melbourne PHN GPs Can Access POLAR
