Model of care:
Clinicals by Outcome Health offers a multi-disciplinary and multistage model of care.
This involves a three-step process:
- Initial review and assessment by a Mental Health Nurse Practitioner
- Consultation with a Psychiatrist
- Follow up review and ongoing care planning with a Mental Health Nurse Practitioner
Please note that we are not a crisis management facility.
In an emergency, please ring 000 or go to the Emergency Department at your local hospital.
For non-urgent support, please call:
Mental Health Crisis Line: 1800 011 511
Lifeline: 13 11 14
BeyondBlue: 1300 22 46 36
What should I bring to my consultations?
Please bring:
- any previous Mental Health reports, documentation, diagnosis etc.
- a list of all current medications.
You are welcome to bring an appropriate support person.
What can I expect from my consultations?
Your first consultation will be with a Mental Health Nurse Practitioner (MHNP) or Credentialled Mental Health Nurse (CMHN). The nurse will gather a detailed history and undertake a thorough assessment. This information will be collated and made available for review by our Psychiatrist in subsequent consultation.
The second consultation is with a Consultant Psychiatrist who will review the information from your initial consultation and will undertake a thorough Psychiatric assessment. The outcome from this consultation may result in a diagnosis, treatment plan, medication change or similar dependent on the clinical assessment of the consultant at the time.
The third and any subsequent consultations will include a review, follow up, and, where appropriate, therapy as well as future care planning opportunities.
As you progress through our 3-step Model of care you may wish to proceed with some therapy sessions with our Specialist team. An agreed care plan will be established to guide the ongoing therapy.

We request all patients arrive 10 minutes before their scheduled appointment. This allows us to maintain our commitment to timely and quality care for all patients.
If a patient arrives late for their appointment, the following protocol will apply:
- Arrival within 15 minutes of the appointment start time: Patient appointment will be shortened to the remaining treatment time, and the full appointment fee will be charged.
- Arrival more than 15 minutes late: If a patient arrives more than 15 minutes late, we may not be able to provide a clinically appropriate session. In such cases, the appointment will be considered a “no-show,” and the associated fee will be charged.
Collateral History
- A complete mental health assessment may require us to gather a collateral history. A collateral history is information gathered from someone other than the patient, commonly family members, significant others or health professionals.
- Where a collateral history is required and with the permission of the client, an interview with the appropriate person will be carried out by our Mental Health Nurses.
