The CESPHN Liver Toolkit Project introduces Dr David Baker and other specialist health clinicians as they work to improve outcomes for those at high risk of liver cancer. Running continuously since early-2021, this project is a key Central Eastern Sydney Primary Health Network initiative with the aims of:
- Identifying and improving the outcomes of patients living with liver cirrhosis and the most common form of primary liver cancer – hepatocellular carcinoma (HCC)
- Improving working relationships and communication between liver centres and general practice
This project, in partnership with St George Hospital, and the AW Morrow Liver Centre at RPA, as well as other partners including Hepatitis NSW, has used POLAR data in practice to identify 13,000 active patients at risk of Liver Cancer for subsequent referral and follow-up.
